Classes I–VI

I

Class I

Pits and fissures on occlusal surfaces, or in buccal or lingual pits.

Location in a pit or fissure is the deciding feature.
II

Class II

Proximal surfaces of premolars and molars.

The posterior proximal location defines the class; a generic smooth-surface label does not.
III

Class III

Proximal surfaces of anterior teeth without incisal-edge involvement.

If the incisal edge is involved, sort the lesion as Class IV instead.
IV

Class IV

A proximal surface of an anterior tooth with incisal-edge involvement.

Both the anterior proximal surface and the incisal edge are involved.
V

Class V

The gingival or cervical third of a facial or lingual surface.

The cervical-third surface location is the deciding feature.
VI

Class VI

A posterior cusp-tip lesion, or a lesion confined to the incisal edge of an anterior tooth.

An anterior example stays on the incisal edge and does not extend onto a proximal surface.
